MyMedVault - Consent to Share Health Records with a Care Provider
Effective Date: 1 September 2025
Purpose of Consent
By proceeding, you authorise MyMedVault ("MMV") to temporarily allow a care provider to view selected medical records. This supports better healthcare coordination, specialist consultations, and second opinions — all fully controlled by you.
Consent Summary

| Audit Trail | All sessions, timestamps, and Care Provider details (if entered) are logged and visible in your account |
Care Provider Details (Optional)
You may choose to enter the following to personalise your sharing history:
- Care Provider's Name
- Specialisation
- Institution Name
These fields are optional and do not affect the sharing process.
Information Shared
Care providers:
- May only view records within the MMV secure portal
- Cannot copy, screenshot, download, or extract your data
- Must abide by MMV's Terms and Conditions as well as Care Provider Acknowledgement Policy
Security and Notifications
- QR code access triggers an email alert to you
- User ID + DOB access triggers both a push notification and an email alert
- The session duration and event details are saved in your audit log
- Sessions expire automatically after 60 minutes, or can be cancelled manually
Your Rights
- View all access sessions, including care provider and timestamp data
- Revoke active sharing at any time
Risks and Benefits
MMV uses best-in-class encryption and access control, but as with all digital systems, residual risk remains. This is mitigated through time-limited access, and audit logs.
